The City of Coral Springs, in partnership with the U.S. Department of State, Passport Division, offers two convenient locations to apply for a U.S. Passport.

No Appointment Necessary
Passport Photos are available at City Hall in the Mall for $10.00

Passport Fees

Adult Passport Book Fees:
(16 years of age and over)
$110.00 application fee payable to the U.S. Department of State
(check, money order or Travelers Check only)
as well as a $25.00 execution fee payable to the City of Coral Springs
(cash, money order, credit cards (MC/Visa) or Travelers Check)

Child Passport Book Fees:
(15 years of age and under)
$80.00 application fee payable to the U.S. Department of State
(check, money order or Travelers Check only)
as well as $25.00 execution fee payable to the City Of Coral Springs

Expedite Fee:
$60.00 fee for each passport also payable to the U.S. Department of State in addition to the passport and execution fees above (check, money order or Travelers Check only) You will then receive your passport within 3-4 weeks; OR with overnight delivery you will receive your passport in about 2-3 weeks door to door (additional fees and instructions)**

No foreign check or foreign currency will be accepted.

**U.S. Express Mail charges are based on current postal rates. (Fees are made payable to the US Department of State for the return of the U.S. Passport).

SPECIAL REQUIREMENTS FOR CHILDREN

Each minor child applying for a Passport Book or Passport Card MUST appear in person.

AS DIRECTED BY PUBLIC LAW 106-119: - Title 22, Section 51.28
Effective February 1st, 2008
To submit an application for a child under age 16
both parents or the child’s legal guardian(s) must appear and present the following:

Evidence of the child’s U.S. citizenship
(an original birth certificate or an original naturalization certificate)
If the child has an expired passport – they will submit their expired passport as well as their birth certificate – this will show relationship to the parents
Parental identification

If only one parent appears, you must also submit one of the following:

1.1. The non-attending parent must fill out the consent form (DS 3053) and it MUST BE NOTARIZED and a copy of the identification front and back (driver's license, state ID or parent’s passport) MUST BE submitted with the DS 3053. As an alternate to the DS 3053, a notarized consent letter from the absent parent is also acceptable along with a copy of identification front and back (driver’s license, state ID of parent’s passport) stating permission for the minor child to obtain a US Passport. The applying parent will sign the application (DS-11) in front of the clerk after oath is given. Statement cannot be more than 3 months old and must come with a photocopy of the front and back side of the second parent’s identification.

2. If one parent is unable to obtain the consent from the other parent, the form DS-5525 must be completed. Your statement on this form must explain the reason why you can to obtain the notarized statement of consent. You must show that there are exigent or special family circumstances that make the other parent’s/guardian’s consent unobtainable.

3. Primary evidence of sole authority to apply.

4. If a parent is deceased a death certificate needs to be submitted.

NOTE: If any of the above documents are not provided, your application may be delayed by the U.S. Department of State, which could result in extra fees on your part.

FIRST TIME APPLICANTS

Anyone applying for a Passport Book or Passport Card must appear in person, including minor child.

When applying for a U.S. Passport you must bring the following items:

  • Proof of U.S. citizenship, such as an original or certified birth certificate Original Naturalization Certificate.
  • Expired U.S. passport

NEW U.S. BIRTH CERTIFICATE REQUIREMENTS AS OF APRIL 1ST, 2011. The U.S. Department of State will require full names of the applicant’s parent(s) to be listed on all certified birth certificates to be considered as primary evidence of U.S. citizenship for all passport applicants, regardless of age.  Certified birth certificates missing this information will not be acceptable as evidence of citizenship.
In addition to this requirement, certified copies of birth certificates must also include the following information to be considered acceptable primary evidence of U.S. citizenship:

  • Full name of the applicant
  • Date of birth
  • Place of birth
  • Raised, embossed, impressed or multicolored seal of issuing authority
  • Registrar’s signature
  • The date the certificate was filed with the registrar’s office (must be within one year)

* PHOTOS - One (1) 2x2 passport photo in color. Taken within the last 6 months to reflect your current appearance. Photos must show a clear image with a white background (Digitized photographs are acceptable if they meet the same criteria as required for conventional photographs) Photos that are damaged, grainy in appearance, or poorly focused will not be accepted. Vending machine and newspaper photographs are not acceptable. (The face size, measured from the bottom of the chin to the top head (including hair), should not be less than 1 inch, no more than 1 3/8 inches)

Depending on when you want your passport, prices vary.

  • Normal (routine) processing takes 4 to 6 weeks.
    The total for an Adult Passport (16 years or older) is $135.00. This is payable in TWO separate payments (checks, money orders or travelers checks). The first is paying to the U.S. Department of State in the amount of $110.00, this MUST BE a check or money order (starter checks are not acceptable). The second is payable to the City of Coral Springs in the amount of $25.00. (Only the $25.00 fee can be paid with cash or credit cards MC/Visa).

    The total from a child’s passport (15 years and under) is $105.00. This is payable in TWO separate payments. The first is payable to the U.S. Department of State in the amount of $80.00, this MUST BE a check or money order (starter checks are not acceptable). The second is payable to the City of Coral Springs in the amount of $25.00. (Only the $25.00 fee can be paid with cash or credit cards – MC/VISA)

FOR FASTER SERVICE: You may expedite (This takes about 4 weeks).

PLEASE NOTE:  If you are paying by a personal check – your personal check must have the following:  payer’s name and address PRINTED on the face of the check.  If you do not have this on your check you must bring in a money order.
NO STARTER CHECKS WILL BE ACCEPTED

NEW PASSPORT CARDS

The Passport Card may ONLY be used for land and sea travel between the United States and Canada, Mexico, Bermuda, and the Caribbean. 
Customers will now have the option to apply for a Passport Book, a Passport Card or Both.
 
TO APPLY FOR THE PASSPORT CARD YOU WILL STILL NEED TO SUBMIT THE SAME DOCUMENTS THAT ARE REQUIRED FOR A U.S. PASSPORT BOOK.

RENEWAL – U.S. PASSPORT RENEWAL APPLICATION FOR ELIGIBLE INDIVIDUALS (Form DS-82)

This form may ONLY be used if YES is answered to ALL six (6) questions.

  1. I can submit my most recent U.S. passport book and/or U.S. passport card with is application.
  2. I was at least 16 years old when my most recent U.S. passport book and/or U.S. passport card was issued.
  3. I was issued my most recent U.S. passport book and/or U.S. passport card less than 15 years ago.
  4. My most recent U.S. passport book and/or U.S. passport card for which I am renewing has not been lost, stolen, mutilated or damaged.
  5. My U.S. passport book and/or U.S. passport card has not been limited from the normal ten (10) year validity period due to passport damage/mutilation, multiple passport thefts/losses, or non-compliance with 22 C.F.R. 51.41. (Please refer to the back pages of your U.S. passport book for endorsement information).
  6. I use the same name as on my recent U.S. passport book and/or U.S. passport card.

    –OR–

    I have had my name changed by marriage or court order and can submit proper documentation to reflect my name change.

If you answered YES to the questions you are eligible to use the renewal form DS-82 you can go to the State Department website: www.travel.state.gov and download the DS-82 form. CAREFULLY FOLLOW THE INSTRUCTIONS – INCOMPLETE OR UNACCEPTABLE APPLICATIONS WILL DELAY THE ISSUANCE OF YOUR PASSPORT.

If you answered NO to any of the six (6) statements about, STOP – you cannot use this form!! You must apply using application DS-11 by making a personal appearance before an acceptance passport agent, postal clerk or clerk of court authorized to accept passport applications.
